Social Services / Outreach to Homebound

Social Services are offered to Plainville residents 60 years and older through the Plainville Senior Center. The Senior Center Social Worker is available to assist individuals with a myriad of services including, but not limited to: State and Federal programs, financial assistance, insurance forms and applications, food programs, transportation services, energy assistance, support groups, legal aid referrals, Lifeline, File of Life and counseling as needed.

The Outreach Services program is designed to fill some of the gaps in services that occur when someone is homebound and/or isolated in their home most of the time. The Senior Center Social Worker is available to visit homebound seniors and assist them with their individual needs, including but not limited to: financial and insurance issues, medical billing questions, home health and home care opportunities and more to insure that individuals are safe and living as comfortably as possible in their own home.
