| Complete this application to re-new an existing license or to apply for a new foodservice license.

| Complete this application if you are planning to operate a 1-day, 2-day or two-week temporary foodservice operation, such as during a fair, faith-based events and other special events where the public is invited.

| Itinerant Vendors

| Complete this application if you are proposing a mobile foodservice operation, such as a hotdog cart, coffee truck or other mobile set-up. These type operations are not permanently fixed to the location and will need to move at the end of each day. Be advised that prior approval, from the Town Planner and the Police Department, is required to determine if the location, size and hours of the operation are acceptable and safe. Upon approval from those departments, the foodservice part of the operation will be inspected by this department.
